The Importance of Regular Check-Ups: How to Maintain Your Health Maintaining health is a top priority in life; however, it is often overlooked amidst the fast-paced daily routine, where busy lifestyles take over, causing many to postpone regular check-ups and only visit doctors when experiencing concerning symptoms. This is contrary to what doctors recommend regarding regular check-ups. Why Are Regular Check-Ups Important? Studies and medical research indicate that regular check-ups are essential for everyone. Here are six reasons to schedule your check-up at Cleveland Clinic Doha as soon as possible:
  1. Awareness of General Health Many people go through their daily lives unaware of the signals their body sends. This ignorance can lead to the worsening of health issues that could easily be treated. Regular appointments with your doctor keep you informed about your health status, enabling you to take the right steps to maintain your overall well-being.
  2. Prevention of Serious Health Complications Regular check-ups are an effective way to detect diseases in their early stages. For instance, treating high blood pressure early can prevent more serious complications. Your doctor can also notice any bodily changes that indicate health problems, allowing you to address issues before they worsen.
  3. Promoting Healthy Habits Regular visits to your doctor help reinforce healthy habits such as proper nutrition, exercise, and stress reduction. Seeing improvements in your medical test results serves as a significant motivation to maintain your health and adhere to these habits.
  4. Keeping Your Body in Excellent Condition Regular check-ups at Cleveland Clinic Doha help ensure your body remains in top shape. Your doctor will provide recommendations to maintain your physical health, which can reduce issues like hormonal imbalances and frequent high blood pressure.
  5. Strengthening the Immune System Individuals who undergo regular check-ups are less prone to illnesses due to adopting healthy habits that boost their immune systems.
  6. Cost Savings Some may hesitate to get regular check-ups due to the belief that they are costly. However, regular check-ups can lead to significant savings in the long run by reducing the risk of developing serious diseases that require expensive treatments.
How Often Should You Get Regular Check-Ups? Recommendations for regular check-ups vary by age, risk factors, and current health status. Generally, it is advised to have check-ups as follows:
  • Every three years for those under 50 in good health.
  • Annually after age 50.
  • More frequently if chronic conditions such as diabetes or chronic obstructive pulmonary disease are present.
Regular check-ups are not just a routine procedure; they are an investment in your health. Schedule your appointment at Cleveland Clinic Doha today and start maintaining your health and well-being.

Dental implants are a popular solution for replacing missing teeth, but they come with several potential drawbacks. Here’s a detailed look at some of the main disadvantages of dental implants.

What Are the Drawbacks of Dental Implants?

  1. High Cost
    • One of the most significant drawbacks of dental implants is their cost. They can be quite expensive, and often insurance does not cover them. It’s advisable to discuss the full costs with your dentist beforehand and explore potential financing options. (1)
  2. Not Suitable for Everyone
    • Certain health conditions or factors may make you unsuitable for dental implants. Eligibility is determined based on a thorough evaluation by your dentist. Conditions that might exclude you include: (2)
      • Being under 18 years old, as jawbone growth may not be complete.
      • Insufficient bone density in the jaw to support the implant.
      • Severe tooth decay.
      • Difficulty maintaining oral hygiene.
      • Untreated gum disease.
      • Smoking, which can adversely affect healing.
      • Certain medical conditions, such as autoimmune disorders or bone disorders.
  3. Surgical Procedure Required
    • Dental implants require surgery to place the implant and, in some cases, bone grafting if there is insufficient bone support. Potential complications of the surgery include: (3)
      • Infection at the implant site.
      • Damage to neighboring teeth.
      • Nerve damage, which could cause pain or numbness in the teeth, gums, or lips.
      • Bleeding.
      • Issues with the sinuses, particularly with upper jaw implants.
Although these complications are uncommon, they can be managed with proper care and adherence to your dentist’s instructions.
  4. Not an Immediate Solution
    • Dental implants involve multiple procedures and require time for healing and integration. For instance, any existing dental or gum issues must be addressed first, followed by evaluation for bone grafting if needed. Once the implant is placed, you typically wait 4 to 8 months for the implant to fuse with the jawbone before the permanent crown is attached. (3)
However, there have been advances in immediate-load implants, and you can consult your dentist about whether this option is suitable for you.
  5. Risk of Implant Failure
    • In some cases, the implant may not properly integrate with the jawbone, leading to implant failure or the need for removal. If this occurs, you may need to repeat the procedure or explore other alternatives. (4)

Circumcision is a common procedure performed on male infants to remove the foreskin from the end of the penis for religious, cultural, social, or medical reasons. Although the procedure is relatively straightforward, it can be painful and may require general anesthesia in some cases. This article explores the details, benefits, and drawbacks of using general anesthesia for circumcision in children.

General Anesthesia for Pediatric Circumcision

Circumcision is often performed on the first or second day after birth. While the procedure is relatively simple, it is painful and comes with some potential complications, necessitating anesthesia for the child. In most cases, local anesthesia is used to numb the area where the surgical incision is made. Local anesthetics can include topical creams applied to the penis or injections around the penis. However, for older children and adults, general anesthesia is recommended due to the complexity of the procedure and the need for complete immobility. For children over one year of age, general anesthesia is strongly advised as it ensures the procedure is carried out safely and effectively. Local anesthesia may still be used to make the initial recovery period as comfortable as possible.

Benefits of General Anesthesia for Pediatric Circumcision

General anesthesia offers several advantages for pediatric circumcision:
  • Calming Effect: General anesthesia helps sedate the child, reducing their movement and making the procedure easier and safer for the surgeon.
  • Pain Relief: The child will not feel pain during the procedure and will not remember the experience.
  • Safety and Stability: Ensures the child remains still and secure during the operation, which enhances safety and effectiveness.

Drawbacks of General Anesthesia for Pediatric Circumcision

Despite its benefits, general anesthesia carries certain risks:
  • Respiratory Depression: The medications used can depress breathing, which poses risks, particularly for newborns or those with certain health conditions.

Important Considerations for General Anesthesia

Here are key details about preparing for and undergoing general anesthesia for circumcision:

Pre-Anesthesia Preparation

  • For Children Over 12 Months:
    • Avoid giving solid foods or non-clear liquids, including milk, formula, juices with pulp, candies, and gum after midnight the night before the surgery.
    • Clear liquids like water and apple juice can be given until two hours before arriving at the hospital.
  • For Infants Under 12 Months:
    • Formula can be given up to six hours before the hospital visit.
    • Breast milk can be given up to four hours before arrival.

During Anesthesia

  • The child will receive anesthesia through an intravenous line, a breathing mask, or both. The anesthesia will be maintained throughout the surgery to keep the child asleep.
  • Vital signs such as heart rate, blood pressure, body temperature, and oxygen levels will be continuously monitored. An endotracheal tube may be used to assist with breathing.

After Anesthesia

  • Post-surgery, the child will be moved to a recovery room to monitor vital signs. The length of stay in recovery varies as some children take longer to wake up from general anesthesia.
  • Upon waking, children may react with crying, discomfort, confusion, nausea, or vomiting.

Benefits of Circumcision

Circumcision has several benefits, including:
  • Prevention of Urinary Tract Infections: Reduces the risk of infections in infants.
  • Lower Risk of Penile Cancer: Reduces the risk of penile cancer in adult males.
  • Reduced Risk of Sexually Transmitted Infections: Lowers the risk of some sexually transmitted diseases.
  • Prevention of Penile Infections or Swelling: Minimizes the risk of infections or inflammation of the penis.
For a safe circumcision procedure, you can trust the Cleveland Clinic Doha, where the Pediatric and Neonatology Department is equipped with advanced technologies and overseen by a highly skilled medical team with local and international expertise.

Diabetic foot is a common condition affecting 19-34% of people with diabetes worldwide, who number around 537 million. However, proper blood sugar management and daily foot care can significantly reduce the risk of developing this condition.

What is Diabetic Foot?

Diabetic foot encompasses a range of problems that can affect a diabetic patient’s feet. Diabetes can lead to complications affecting the nerves and blood vessels in the feet, making them more susceptible to injuries (such as infections or wounds) and slower to heal. If not treated promptly, these issues can worsen and may lead to tissue death (gangrene).

Causes of Diabetic Foot

Chronic high blood sugar levels lead to nerve damage, poor blood circulation, and weakened immune function. This results in reduced sensation in the feet, making it difficult to notice any injuries or wounds and slowing their healing. This increases the risk of infection and its rapid spread. If not addressed immediately, it can lead to more severe complications.

Symptoms of Diabetic Foot

If you have diabetes and notice any of the following symptoms, consult your doctor immediately:

  • Nerve Problems: Numbness, tingling, or decreased pain sensation over time, increasing the risk of unnoticed injuries.
  • Pain or Cramping: Burning or discomfort in the feet, especially during physical activity or rest.
  • Wounds or Ulcers: Slow-healing wounds or sores from minor injuries or pressure.
  • Skin Changes: Redness, warmth, or color changes in the skin of the feet.
  • Loss of Temperature Sensation: Inability to feel heat or cold.
  • Dry or Cracked Skin
  • Calluses or Thickened Skin: Possibly with dried blood inside.
  • Ingrown Toenails
  • Thick, Yellow Toenails
  • Hair Loss: On the feet and toes.
  • Foot Deformities: Changes in the shape of the foot over time.
  • Infections: Including fungal infections like athlete’s foot, which may be black or foul-smelling.

Prioritize your foot health today! If you experience any of these symptoms or other unusual signs, don’t wait—schedule an appointment with the Endocrinology and Diabetes Department at the Cleveland Clinic Doha for early detection and effective management of diabetic foot complications.

Diagnosis of Diabetic Foot

Diagnosing diabetic foot includes:

  • Medical History Review: General health, duration of diabetes, and any previous foot problems.
  • Clinical Examination: Looking for signs of nerve damage, poor blood circulation, and any existing wounds or ulcers using tools like a tuning fork and ultrasound.
  • Blood Tests: Including glucose tests, especially HbA1c to assess diabetes management over the past 2-3 months, and a complete blood count (CBC) to evaluate infection markers.
  • Imaging Tests: Such as X-rays to detect fractures or dislocations, and MRI or CT scans to assess soft tissue damage, infections, or bone deformities.

Treatment of Diabetic Foot

Treatment depends on the severity of the condition and involves a comprehensive approach:

  • In Mild and Early Cases:
    • Wound Care: Clean the wounds thoroughly, apply dressings, and remove dead tissue (debridement).
    • Special Footwear: Use shoes or orthotics to reduce pressure on affected areas and promote healing.
  • In Cases of Infection:
    • Pain Relief: Nerve pain medications.
    • Antibiotics: Topical, oral, or intravenous antibiotics.
    • Surgical Drainage: For severe cases, regular dressing changes and surgical debridement may be necessary.
  • In Advanced Stages:
    • Advanced Treatments: Antibiotics, hyperbaric oxygen therapy, and blood thinners to improve blood flow.
    • Surgical Intervention: If necessary, to treat blood vessels, remove damaged tissue, or even amputation in extreme cases, but only as a last resort.

Complications of Diabetic Foot

Potential complications include:

  • Skin and Bone Infections
  • Tissue Death (Gangrene)
  • Charcot Foot: Bone damage and deformities due to neuropathy.
  • Amputation: In severe, non-responsive cases where ulcers do not heal.

Prevention of Diabetic Foot Complications

Controlling blood sugar levels and daily foot care are the best measures to prevent diabetic foot issues. Good foot care for diabetics includes:

  • Manage Blood Sugar: Through medication, diet, and exercise.
  • Daily Foot Inspection: Check for wounds or signs of infection and see a doctor if any are found.
  • Foot Hygiene: Wash feet daily with warm water and soap, and dry them thoroughly, especially between the toes.
  • Nail Care: Trim nails straight across and smooth sharp edges; seek professional help if needed.
  • Comfortable Footwear: Wear well-fitting shoes that provide support and protect the feet. Avoid ill-fitting shoes and walking barefoot.
  • Moisture-Absorbing Socks: Wear cotton socks of the right size.
  • Quit Smoking: Smoking impairs circulation and increases the risk of complications.
  • Avoid Barefoot Walking: Especially outdoors, and even indoors if possible.
  • Regular Check-ups: Have your feet examined by a doctor regularly.
  • Control Other Health Conditions: Such as high blood pressure and cholesterol.

Critical Care Cases in Intensive Care Unit (ICU)

Critical care cases in the ICU are some of the most complex and life-threatening conditions, requiring continuous and specialized care around the clock from a dedicated medical team. These cases encompass a broad range of severe health issues that endanger a person’s life.

What are Critical Cases in the ICU?

Critical cases in the ICU refer to conditions that require intensive medical care and constant monitoring due to their severity and significant impact on bodily functions. The ICU is designed to manage these situations to ensure patient survival and minimize complications.

Examples of Critical Cases in the ICU

Some examples of critical cases in the ICU include:

  • Sepsis: A severe infection that spreads throughout the body, leading to systemic inflammation and organ failure.
  • Severe Stroke: Significant impairment of brain function due to disrupted blood flow to the brain.
  • Acute Respiratory Diseases: Such as severe pneumonia or critical bronchial obstruction.
  • Acute Cardiac Conditions: Including severe heart attacks or aortic dissection (a tear in the aorta).
  • Severe Trauma: Resulting from accidents, including severe head injuries, extensive bone fractures, or critical spinal cord injuries.
  • Circulatory Shock: Caused by reduced or halted blood flow and oxygen to vital organs, such as shock from severe burns or major bleeding.
  • Severe Toxicity: Including severe chemical or food poisoning.
  • Organ Failure: Where organs fail to perform their normal functions due to severe infections, chronic diseases, such as kidney or heart failure.
  • Postoperative Monitoring: Intensive monitoring after major surgeries.

Management of Critical Cases in the ICU

Management of critical cases in the ICU involves:

  • Monitoring Vital Signs: Using specialized equipment to continuously monitor heart rate, breathing rate, blood oxygen levels, body temperature, and urine output.
  • Continuous Monitoring: Often, one nurse is assigned to each patient or to every two patients in the ICU to ensure close and continuous monitoring.
  • Specialized Equipment: Depending on the severity of the condition, equipment used may include:
    • Mechanical Ventilation: To assist with breathing.
    • Intravenous (IV) Lines: For administering fluids, glucose, medications, and nutrition.
    • Feeding Tubes: For patients unable to eat by mouth.
    • Drains: To remove excess fluids or blood from the body.
    • Catheters: For draining urine.
  • Medications: Most ICU patients receive pain relief or sedatives to manage discomfort from the equipment and procedures.

What Happens After Leaving the ICU?

Once a patient’s condition stabilizes, they are moved to a regular hospital room, with the duration of stay ranging from a few days to several months depending on the severity of the condition. Although many patients recover fully, some may experience:

  • General Weakness
  • Post-Traumatic Stress Disorder (PTSD): Feelings of anxiety and stress.
  • Depression
  • Difficulty with Concentration or Memory
  • Sleep Problems
  • Muscle Stiffness
  • Loss of Appetite
  • Weight Loss
  • Overall Fatigue and Exhaustion

The ICU department at our hospital features a highly skilled medical and nursing team trained specifically to handle all critical care cases with meticulous attention and continuous monitoring.

Gynecomastia is the enlargement of breast tissue in males, which can occur during certain life stages such as puberty, lactation, and aging due to hormonal changes. This condition can cause breast pain and significant social anxiety. The breasts may grow unevenly. In most cases, gynecomastia resolves within 12 months. However, if the condition persists, treatment by a specialized team, including plastic surgeons and endocrinologists, may be recommended.

Causes of Gynecomastia

In the majority of cases, gynecomastia is idiopathic, meaning the cause is unknown. It can be caused by increased estrogen activity, decreased androgen activity, or a combination of both.

Certain factors that can contribute to gynecomastia include:

  • Steroid Use: Use of anabolic steroids can affect hormone levels.
  • Obesity: Increased body fat can elevate estrogen levels.
  • Medications: Some antibiotics, antidepressants, and other drugs.
  • Liver or Kidney Diseases: These conditions can increase the production of androstendione by the adrenal glands.
  • Marijuana Use: Cannabis use may influence hormonal balance.
  • Tumors: Tumors of the testicles, adrenal glands, or pituitary gland.
  • Malnutrition: Poor nutrition can lead to reduced levels of gonadal hormones.
  • Hypogonadism: Reduced function of the testes.
  • Type 1 Diabetes: This can affect hormone levels.
  • Neurofibromatosis: Rarely associated with gynecomastia.
  • Genetic Disorders: Such as Kennedy’s Syndrome.
  • Stress and Anxiety: May contribute to hormonal imbalances.

Risk Factors for Gynecomastia

Certain factors increase the likelihood of developing gynecomastia:

  • Age: More common in adolescents and men over 60.
  • Genetic Factors: Conditions like Klinefelter syndrome.
  • Medications: Use of anti-androgens.
  • Medical Conditions: Kidney and liver disease, hypogonadism.

Symptoms of Gynecomastia

Signs and symptoms of gynecomastia may include:

  • Breast Enlargement: Sometimes asymmetric.
  • Pain or Discomfort: In the breast area.
  • Nipple Discharge: Possible fluid from the nipples.
  • Body Image Issues: Psychological impact due to changes in appearance.

Diagnosis of Gynecomastia

Diagnosis is primarily based on medical history and physical examination, including a full-body check for other abnormalities. Evaluation by an endocrinologist may include:

  • Blood Tests: To assess thyroid, liver, and kidney function, tumor markers, and hormone levels.
  • Imaging Studies: Mammography or ultrasound to evaluate breast tissue abnormalities.

Prevention of Gynecomastia

Preventive measures include:

  • Healthy Lifestyle: Maintaining a balanced diet and avoiding excessive alcohol consumption.
  • Managing Stress and Blood Sugar Levels: Keeping stress and diabetes under control.
  • Medication Caution: Avoiding non-prescribed medications and consulting a doctor before starting any new medication.

Treatment of Gynecomastia

Treatment options vary depending on the underlying cause:

  • Addressing Underlying Causes: If gynecomastia is due to a specific factor like medication, altering or stopping the medication may resolve the issue.
  • Hormonal Imbalances: Treating any hormonal imbalances or medical conditions that are contributing to the problem.
  • Monitoring: If gynecomastia is recent and changes within 12 months, monitoring might be recommended as it may resolve on its own.
  • Surgical Options: For gynecomastia persisting for more than 12 months, surgical interventions such as mastectomy or liposuction may be suggested.

Fistulas vary based on their location. Here are the main types:

Anal Fistula

An anal fistula is an abnormal connection between the end of the anus and the surrounding skin. It usually appears as a small hole or painful red swelling near the anus. Anal fistulas can lead to complications such as fecal incontinence or pain during bowel movements. Causes:
  • Bacterial infection
  • Blockage of the anal glands
  • Inflammatory conditions like inflammatory bowel disease
  • Sexually transmitted infections affecting the anus
  • Cancer or damage from radiation therapy for cancer
  • Injury or trauma during surgery

Vaginal Fistula

A vaginal fistula can form near the reproductive, digestive, and urinary systems. Types include:
  • Vesicovaginal Fistula: Connects the bladder and the vagina.
  • Uretero-vaginal Fistula: Connects the ureter and the vagina.
  • Rectovaginal Fistula: Connects the rectum and the vagina.
This type can occur due to vaginal damage during childbirth, a cesarean section, or other surgeries. It may also develop from sexually transmitted infections. Additionally, it can result from cancer in the colon, uterus, or bladder, or from radiation used to treat these cancers. Symptoms:
  • If connected to the bladder, it may cause urinary incontinence.
  • If connected to the rectum, it may lead to frequent urinary tract infections, uterine infections, or foul-smelling vaginal discharge.

Tracheoesophageal Fistula

This type connects the trachea (windpipe) with the esophagus, the tube that carries food from the mouth to the stomach. It allows food and liquids to pass from the esophagus into the trachea and lungs, leading to choking, coughing, or turning blue when eating or drinking. Often congenital, this type of fistula may also result from surgical injuries or trauma from intubation.

Arteriovenous Fistula

An arteriovenous fistula causes abnormal blood flow between blood vessels. Typically congenital, it can occur in the legs, arms, kidneys, brain, or lungs. Symptoms of Fistulas: Symptoms vary based on the fistula’s location and range from mild to severe. Common symptoms include:
  • Pain around the anus
  • Persistent urinary leakage from the vagina
  • Malaise
  • Irritation of external genitalia
  • Nausea
  • Abdominal pain
  • Recurrent urinary tract infections
  • Vomiting
  • Fever
  • Diarrhea
  • Weight loss
  • Constipation
How to Treat Fistulas Some small fistulas may close on their own without causing significant symptoms. Others can cause pain or severe infections and require treatment. For Arteriovenous Fistulas: If small, treatment may involve special compression garments or gloves to apply pressure to the blood vessels and allow healing, along with medications to prevent blood clotting. Depending on the location and type of fistula, treatment options may include:
  • Surgical Procedures: To repair or close the fistula.
  • Catheterization: Inserting a tube to allow drainage and self-healing of the fistula.
  • Injection of Materials: To block the arteriovenous fistula.
  • Radiation Therapy: For closing small fistulas.
If left untreated, some fistulas can lead to severe infections and life-threatening conditions.

What is Cystoscopy?

Cystoscopy is a minimally invasive procedure used to examine the bladder and urethra for any issues. It involves inserting a thin tube with a camera, called a cystoscope, through the urethra into the bladder. Additionally, surgical tools can be inserted through the cystoscope to treat conditions during the examination. (1)

Types of Cystoscopy

  1. Flexible Cystoscopy: Uses a thin, flexible cystoscope similar to a pen, which can bend to inspect the entire bladder and urethra. This type is usually performed in an outpatient clinic. (2)
  2. Rigid Cystoscopy: Involves a rigid, metal cystoscope and is used for specific procedures like taking biopsies or injecting medication. This type is typically performed in a hospital under general anesthesia. (3)

Indications for Cystoscopy

A doctor may recommend cystoscopy if you experience urinary symptoms such as: (4)(5)
  • Urinary retention or incontinence
  • Severe pain during urination
  • Blood in the urine
  • Recurrent urinary tract infections
  • Overactive bladder
Cystoscopy helps diagnose or treat various urinary system issues, including: (4)(5)
  • Bladder or urethral cancer
  • Bladder stones
  • Bladder inflammation
  • Urinary tract infections
  • Bladder control disorders
  • Ureteral stricture or fistula
  • Benign prostatic hyperplasia
If you experience any urinary disorders, consult our specialized urology team at Cleveland Clinic Doha. Our experts are skilled in diagnosing and treating various urinary conditions, including performing cystoscopy.

Steps of Cystoscopy

Cystoscopy is a simple procedure that takes about 15-20 minutes and involves: (6)
  1. Urinating to empty the bladder.
  2. Using local or general anesthesia, depending on the type and purpose of the procedure.
  3. Inserting a thin cystoscope with a camera through the urethra into the bladder.
  4. The doctor examines the bladder and urethra on a nearby computer screen.
  5. The bladder may be filled with water or saline to expand it for better visibility.
  6. The doctor might perform additional procedures, such as taking a biopsy.
Post-procedure symptoms may include mild discomfort, frequent urge to urinate, or a small amount of blood in the urine. These symptoms generally last for one or two days and can be relieved by drinking plenty of fluids, using pain relievers as needed, or applying warm compresses to the urethra. (4)(7) You can usually go home the same day and resume normal activities the following day. (7)

Preparing for Cystoscopy

Preparation may vary based on the type and purpose of the cystoscopy. General instructions may include: (4)(5)
  • Taking antibiotics before and after the procedure, especially if you have frequent urinary tract infections.
  • Fasting the night before the procedure if general anesthesia is used.
  • Informing your doctor about all medications you are taking.
  • Stopping certain blood-thinning medications as directed by your doctor.
  • Providing a urine sample on the day of the procedure to check for any infections.
  • Urinating right before the procedure to ensure the bladder is empty.
Women should inform their doctor if they are pregnant, as some anesthesia medications may affect the fetus.

Risks of Cystoscopy

Cystoscopy is generally safe, but like any medical procedure, it carries some risks. Rare complications may include: (6)(8)
  • Bladder or urinary tract infection
  • Bleeding from the biopsy site
  • Bladder perforation
  • Anesthesia-related complications
Contact your doctor if you experience the following symptoms after cystoscopy: (4)(5)
  • Severe pain at the urethra or during urination lasting more than two days
  • Excessive bleeding in the urine
  • Signs of infection (fever, chills, cloudy or foul-smelling urine)
  • Reduced urine output or inability to urinate

Cystoscopy Results

Depending on the type of cystoscopy, the doctor may be able to diagnose the issue immediately and suggest the next steps, including further tests or treatments. If biopsies were taken, results might take some time to be available. (9)
